The Year Five Test
There is a photograph taken at every hospital handover. The ribbon, the officials, the new building shining behind them. Nobody frames the photograph from year five.
Yet year five is where healthcare infrastructure is actually judged. The equipment has been through thousands of cycles. The engineers who commissioned it have moved on. The budget line that built it has closed, and the facility lives or dies on maintenance contracts, spare parts, training, and whether anyone planned for any of that.
Walk through facilities across the developing world and you find the failures of the ribbon-cutting standard: imaging suites idle for want of a part, wards built without the staff to run them, systems nobody was trained to use after the vendor flew home.
The fix is not complicated, but it has to be priced in from the start. Service contracts with named response times. Spare parts logistics. Training that outlives staff turnover. Financing structures that cover operation, not just construction. A single accountable partner who is still answering the phone in year five.
